SANTA ANA - ANAHEIM - ORANGE- IRVINE- WESTMINSTER - GARDEN GROVE AND SURROUNDING AREAS

FULL TIME OR PARTIME AVAILABLE

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team as a ONE ON ONE Shift Care (PRIVATE DUTY) Home Health LVN . In this role, you will provide exceptional care and support ONE chronically ill, but stable patient in the comfort of their own homes. Your primary responsibility will be to deliver high-quality nursing services during assigned shifts, ensuring the well-being and safety of our patients.

• Perform comprehensive assessments of patients' health status and develop individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor vital signs and observe any changes in patients' condition
• Assist patients with personal care activities, including bathing, grooming, and dressing
• Educate patients and their families on managing illnesses, medications, and home care procedures
• Collaborate with friendly Case Managers to ensure coordinated and effective patient care
• Maintain accurate and up-to-date electronic medical records
• Adhere to all applicable laws, regulations, and ethical standards

​ Qualifications:

• Graduate of an accredited school of Nursing
• Possess a valid, current, unrestricted LVN license able to practice in CALIFORNIA - New grads welcome!
• Strong knowledge of medical terminology, tools, equipment and nursing principles
• Prior experience with G-tube, Trach, Home Vent a plus
• Ability to work independently, collaboratively and make sound decisions
• Compassionate and caring demeanor
• Excellent written and verbal communication skills
• Strong organizational and time management skills
• Reliable transportation and willingness to travel to ONE patient's home
