Statement of Purpose: 

The Mobile PrEP Registered Nurse (RN) provides professional nursing care and direct patient services as part of CAN Community Health’s innovative at-home HIV prevention program. This role is responsible for delivering long-acting injectable (LAI) PrEP, conducting rapid HIV testing, performing phlebotomy and lab collection, and ensuring safe and compassionate care for patients in their homes or designated mobile unit locations. The Mobile PrEP RN works closely with providers, the Virtual Care Team, and other interdisciplinary staff to enhance patient access to PrEP, reduce barriers to HIV prevention, and promote patient-centered care in the community.

Essential Functions:

  1. Administers long-acting injectable (LAI) PrEP medications in patient homes or mobile units, following CAN protocols and provider orders.
  2. Performs rapid HIV testing and venipuncture, processes and transports specimens in accordance with laboratory and CAN policy.
  3. Completes required safety assessment prior to at-home visits to ensure patient eligibility and safety.
  4. Provides patient education on PrEP, HIV prevention, medication adherence, follow-up visits, and available support services.
  5. Responds appropriately to positive/reactive HIV tests, provides counseling, initiates confirmatory testing, and connects patients to care.
  6. Collaborates with providers, telehealth clinicians, and the Virtual Care Team (VCT) to ensure appropriate scheduling of at-home PrEP visits.
  7. Confirms telehealth provider appointments are completed prior to injection visits, per CAN policy.
  8. Schedules follow-up appointments, including injection and telehealth visits, and ensures documentation in EMR.
  9. Coordinates with pharmacy teams, including buy-and-bill processes, to ensure medication availability and compliance.
  10. Ensures the mobile unit is fully stocked and operational, including refrigeration, medication security, and infection control compliance.
  11. Follows CAN protocols for no-show patients, including conversion to in-clinic care if criteria are met.
  12. Maintains accurate, timely documentation in EMR, ensuring compliance with CAN, HRSA, Ryan White, and 340B requirements.
  13. Practices safety and infection control procedures per OSHA, CAN policy, and standard nursing protocols.
  14. Acts as a liaison between patients and CAN’s clinical teams to reduce stigma and improve engagement.
  15. Participates in community education and awareness efforts related to HIV prevention, PrEP, and CAN’s mission.

Requirements:

Education/Professional:

  1. Graduate of an accredited school of nursing; Associate or Bachelor of Science in Nursing required.
  2. Current RN license in state of practice.
  3. Basic Life Support (BLS/CPR) certification required.
  4. Valid driver’s license.

Work Environment:

This position operates in mobile medical units, and community locations. Requires flexibility to work in varying environments and maintain compliance with safety, infection control, and confidentiality standards. May involve use of standard office and medical equipment including computers, EMRs, refrigeration units, injection/IV supplies, and phlebotomy tools.

  2. Sedentary- Exerts up to 20 lbs. of force occasionally and/or a negligible amount of force frequently or constantly in lift, carry, push, pull, or otherwise move objects. Involving sitting most of the time but may involve walking or standing for brief periods of time.
  3. Requires expressing or exchanging ideas by means of spoken word, visual and auditory acuity.

Position Type/Expected Hours of Work:

This position requires hours of work outside of normal operating hours. This position is primarily field based, requiring daily travel to patient homes in mobile units, and community locations.

Travel: 

Travel hours are varied and may often be outside of normal CAN business operating hours. Must be able to operate a motor vehicle and have valid insurance and driver’s license.
